NMEA offers three awards, as described on the Awards page. To nominate an individual for any of these awards, use the form on this page. Alternately, you may print out and follow these instructions. Please review the form and instructions carefully and prepare and assemble ALL information and documents in advance so that the application can be completed in one session. It is not possible to save it and come back later for edits or additional uploads. Note that all on-line applications must include the form on this page and a narrative statement (MS Word file, created and uploaded by you). Names of all files to be uploaded should begin with the nominee's name, followed by an indication of its contents (ex: JaneDoeNarrative.doc). File names should not contain spaces.

About the Narrative

The narrative is required and must be a MS Word (.doc or .docx) or PDF file, and can contain no more than 2,000 words total. The file can contain small images, charts, etc in addition to text. Every word in the document counts. MS Word will count them for you. Documents with word counts higher than 2,000 will not be accepted. Please certify on this form that the word count is under 2,000.

About Supporting Documentation

Supporting documents are optional, but you may include up to 3 if you wish. These documents should be MS Word (.doc or .docx) or PDF files and may include: An additional narrative description of the nominee's background as it relates to the award (500 words maximum)Maximum of 5 separate items representing the nominee's products, programs and accomplishments. Examples include curriculum vita, abstracts of publications or educational resources authored by the nominee, news articles about nominee's projects, etc. Maximum of 3 letters of support from colleagues, students, supervisors, etc.

File size maximum is 30MB each. 

The first few fields will automatically fill in with your information as the Nominator, but you can edit that if you wish.
